CArE accepts referrals for individuals living with arthritis and related musculoskeletal conditions who may benefit from assessment, education, treatment, and self-management support. Our goal is to provide timely, patient-centred care through an interprofessional team approach.
Referrals help ensure patients are connected with the most appropriate services and programs based on their individual needs. Depending on the referral, patients may be scheduled for assessment, education sessions, exercise programs, or individualized treatment support.

How to Access CArE Services
Most CArE services and programs require a referral from a healthcare provider. Referrals may be submitted by:
- Family physicians
- Nurse practitioners
- Specialists
- Other healthcare providers involved in your care
Once a referral is received, our team will review the information and determine the most appropriate next steps. Patients may then be contacted regarding appointments, assessments, or program availability.
Referral Services May Include:
- Arthritis assessment and treatment
- Urgent Assessment Clinic referrals
- Group education and exercise programs
- Fibromyalgia, osteoarthritis, osteoporosis, and inflammatory arthritis programs
- 1:1 consultations and individualized support
- Self-management education and resources
Urgent Assessment Clinic Referrals
CArE also offers an Urgent Assessment Clinic for patients experiencing acute inflammatory arthritis symptoms or urgent rheumatologic concerns.
Appropriate urgent referrals may include:
- Suspected rheumatoid arthritis or autoimmune inflammatory arthritis
- Acute crystal arthritis
- PMR, GCA, or vasculitis
- Acute inflammatory flares requiring urgent assessment
