  • What conditions does CArE support?

    CArE supports individuals living with a variety of arthritis and musculoskeletal conditions, including osteoarthritis, inflammatory arthritis, osteoporosis, fibromyalgia, and related conditions. Our programs focus on education, treatment, exercise, and self-management support.

  • I have arthritis / symptoms – Learn about our programs

    If you are experiencing arthritis symptoms or have recently been diagnosed, CArE offers a variety of programs and services designed to help you better understand and manage your condition.

    Our team provides:

    • Assessment and treatment support
    • Education programs focused on arthritis management
    • Exercise and rehabilitation programs
    • Self-management tools and resources
    • 1:1 consultations and group sessions

    Programs are available for conditions including osteoarthritis, osteoporosis, fibromyalgia, and inflammatory arthritis.

  • Do I need a referral to access CArE services?

    Most CArE programs and clinical services require a referral from a healthcare provider. If you are unsure whether a referral is needed, please contact our team and we will be happy to guide you.

  • I have been referred – What to expect

    Once we receive your referral, our team will review the information and determine the most appropriate program or service based on your needs. You may be contacted by a member of our team to schedule an appointment, education session, or assessment.

    Depending on your condition and referral, your care may include:

    • Individual assessment and treatment
    • Group education or exercise programs
    • Support from an interprofessional team, including rheumatologists and allied health professionals
    • Self-management education and resources

    Some services may be offered in person or virtually. If additional information is required before your appointment, our team will contact you directly.

    Supporting someone living with arthritis can feel overwhelming, but you are not alone. CArE offers a variety of educational programs, trusted resources, and support tools to help patients, families, and caregivers better understand and manage arthritis.

    Our resources include:

    • Arthritis education and self-management information
    • Exercise and wellness resources
    • Trusted external organizations and educational websites
    • Group programs and workshops
    • Guidance from our interprofessional care team

    We encourage family members and caregivers to participate in learning opportunities when appropriate, as education and support can play an important role in managing chronic conditions.

  • Are programs offered virtually or in person?

    Some programs and education sessions are offered virtually, while others are delivered in person depending on the type of program and clinical needs. Program format details will be shared with participants when appointments are scheduled.

  • What should I bring to my appointment or program?

    You may be asked to bring a list of medications, relevant medical information, comfortable clothing for exercise-based sessions, and any completed forms provided by our team. Specific instructions will be shared before your appointment.

  • Is there a cost for programs or services?

    CArE services are funded through the Ontario Ministry of Health. If there are any costs associated with a specific program or service, this will be communicated in advance.
