Most CArE programs and clinical services require a referral from a healthcare provider. If you are unsure whether a referral is needed, please contact our team and we will be happy to guide you.

Some programs and education sessions are offered virtually, while others are delivered in person depending on the type of program and clinical needs. Program format details will be shared with participants when appointments are scheduled.
You may be asked to bring a list of medications, relevant medical information, comfortable clothing for exercise-based sessions, and any completed forms provided by our team. Specific instructions will be shared before your appointment.
CArE services are funded through the Ontario Ministry of Health. If there are any costs associated with a specific program or service, this will be communicated in advance.
