Before Your Appointment

Please ensure that we have received your referral and any requested documentation prior to your appointment. If additional information is required, a member of our team may contact you directly.

You may also wish to:

  • Write down any questions or concerns you would like to discuss
  • Prepare a list of current medications and supplements
  • Bring relevant medical information or imaging reports if requested

What to Bring

Depending on the type of appointment or program, you may be asked to bring:

  • Your health card
  • A current list of medications
  • Comfortable clothing and supportive footwear for exercise-based sessions
  • Any forms or questionnaires completed prior to your visit
  • Mobility aids or assistive devices if applicable

During Your Visit

Your appointment may include assessment, education, exercise instruction, or discussion with one or more members of our interprofessional team, which may include rheumatologists, physiotherapists, occupational therapists, pharmacists, and health educators.

Our goal is to provide patient-centred care that supports understanding, symptom management, and long-term wellbeing.

Virtual Appointments & Programs

Some appointments and education sessions may be offered virtually. If your appointment is virtual, instructions on how to join the session will be provided in advance.

Please ensure:

  • You have access to a reliable internet connection
  • Your device is charged and ready before the session
  • You are in a quiet and private space when possible

Appointment Changes or Cancellations

If you need to cancel or reschedule your appointment, please contact our office as soon as possible. Providing advance notice helps us support other patients waiting for care.
