Your Appointment

We want your experience at CArE to be as comfortable, informed, and helpful as possible. Whether you are attending an assessment, education session, or treatment program, our team is here to support you throughout your care journey.

Please take a few moments to review the information below to help you prepare for your visit.

How to prepare for your appointment

Please ensure that any previously requested lab or X-rays have been completed prior to your visit. Also, please complete all online forms and questionnaires we may send to you.

If additional information is required, a member of our team will contact you directly.

Depending on the type of appointment or program,

We ask you to bring:

  • Your health card
  • A current list of medications and supplements
  • Any forms or questionnaires completed prior to your visit, if applicable
  • Mobility aids or assistive devices if applicable
  • Your eyeglasses, if needed, you will have paperwork to fill out during registration

Please:

  • Wear comfortable clothing and supportive footwear for exercise-based sessions
  • Arrive 15 Mins before your scheduled appointment for check in
  • Check in with the receptionist at the front desk
  • Show your valid Ontario health card
  • Keep in mind that we are a scent free environment

You may also wish to:

  • Write down any questions or concerns you would like to discuss
  • Bring relevant medical information, any Lab/Test results and imaging reports if requested

During Your Visit

Your appointment may include assessment, education, exercise instruction, or discussion with one or more members of our interprofessional team, which may include rheumatologists, physiotherapists, occupational therapists, pharmacists, and health educators.

Our goal is to provide patient-centered care that supports understanding, symptom management, and long-term wellbeing.

Virtual Appointments & Programs

Some appointments and education sessions may be offered virtually. If your appointment is virtual, instructions on how to join the session will be provided in advance.

Please ensure:

  • You have access to a reliable internet connection
  • Your device is charged and ready before the session
  • You are in a quiet and private space when possible

Appointment Changes or Cancellations

If you need to cancel or reschedule your appointment, please call our office at 905-235-9990 or email us at info@carearthritisteam.ca as soon as possible. Providing advance notice helps us support other patients waiting for care.
